‘Significant’ failings: Damning findings after foster children placed with murderer

An internal review revealed significant failings after two foster children were allowed to live with triple killer Regina Arthurell in western Sydney last December. The Department of Communities and Justice failed to act on critical information before closing a risk report, resulting in misconduct proceedings for some staff.
